Description: Cast glass sheet mounted on a wooden base, with dedication in relief between coloured representations of the badges of the two Associations, and a frieze of red poppies along the bottom. A brass plaque bearing the incised inscription sits on the centre of the base.
Inscription: On the glass: "We Will/ Remember/ Them"/ Submariners Association/ Merseyside/ HMS Eaglet/ OBA (= Old & Bold Association) On the plaque: Presented to the Church/ of Our Lady and St Nicholas/ by the Submariners Association/ Merseyside and HMS Eaglet/ OBA on 15th June 2019
