    Bronze rectangular cast plaque with inscription in raised letters. All four sides are indented along most of their length leaving the corners extended . Each corner contains a rosette . Presentation plaque is plain brass with inscription in black lettering. researchers note - this memorial has been moved from its original location and the brass plaque is no longer present and is possibly lost. (March 2019 )
