- Slate tablet, which has the Dyfed Powys Police crest top-centre, and incised white lettering.
The upper half of the tablet has the inscription, in both Welsh and English; the lower half records the names of sixteen former police officers from Dyfed Powys Police's historic Forces who died in World War 1, grouped by the name of the Force for which they had formerly worked (i.e. Breconshire, Carmarthenshire, Montgomeryshire, Cardiganshire, Radnorshire and Pembrokeshire), followed by the affirmation "We Will Remember Them" in both Welsh and English.

(The English inscription on the upper right side reads): DEDICATED TO THE MEMORY OF/ OFFICERS FROM THE CONSTITUENT/ FORCES OF DYFED-POWYS/ WHO GAVE THEIR LIVES IN/ THE GREAT WAR 1914 - 1918.
